What is Formoterol?
Formoterol is a long-acting beta-2-agonist (LABA), which is used as a powder inhaler for continuous therapy of asthma and COPD. It is characterized by an unusually fast action that is noticeable after 1-3 minutes.
Unlike most LABA, Formoterol combines rapid action with long duration (12 hours). The monopreparation is used as a supplement to inhalative corticoids.
Known brand names are Formoterol STADA®, Bufori Easyhaler® and Oxis® Turbuhaler. Formoterol is also part of the combination inhalers Symbicort and Foster.
Active Ingredients & Mechanism of Action
Active ingredient: Formoterol
Formoterol activates the beta-2-receptors in the bronchi:
Active mechanism: 1. Binding to beta-2-adrenoceptors in bronchial muscles Two. Activates the adenylate cyclase and increases cAMP 3. The smooth musculature of the bronchis sleeps 4. The airways are extended
Special features:
- Fast action (1-3 minutes) – unusual for LABA
- Full intrinsic activity (fulagonistic)
- duration of action: 12 hours
- Lipophilic anchoring in the cell membrane
Additional effects:
- Inhibition of mast cell release
- Improving mucosiliary clearance
- Protection against bronchoconstrictive stimuli
Who is it suitable for?
Formoterol mono preparations are suitable for:
- Asthma from level 3 (always together with ICS!)
- COPD continuous therapy
- Prophylaxis of stress asthma
Important – safety note: Formoterol may be used with Asthma NIEMALS alone without inhalative corticoid (ICS). LABA monotherapy at asthma increases the risk of severe exacerbations.
Advantages of the monopreparation:
- Flexible dosage independent of ICS
- Fast action
- Suitable for use before sports
Not suitable as:
- Alone asthma therapy (without ICS!)
- Acute treatment as the only measure
Available Dosages
Formoterol STADA / Bufori Easyhaler:
- 12 μg per single dose
Dosing at Asthma:
- 1 inhalation (12 μg) 2x daily
- If necessary: up to 2 inhalations 2x daily
- Before sport: 1 inhalation 15 minutes before
Dosing at COPD:
- 1 inhalation (12 μg) 2x daily
- Maximum: 48 μg per day
** Always use in combination with an inhalative corticoid!
How to Take
Easyhaler (powder inhaler): 1. Remove protective cap Two. Keep device upright, shake once 3. Press and release (dose is loaded) 4. Breathe deep (not in the inhaler) Five. Close the mouthpiece 6. Strong and deep breathing 7. Keep breath 5-10 seconds 8. Breathe slowly
Important notes:
- Always at the same time (morning and evening)
- Apply regularly
- Do not drop abruptly
- In addition to the inhalative corticoid – never alone!
Contraindications
Do not apply at:
- hypersensitivity to formoterol or lactose
- As sole therapy for asthma (without ICS!)
Preview at:
- Heavy cardiovascular diseases
- QT extension
- Hyperthyroidism
- Diabetes mellitus (can increase blood sugar)
- Hypokaliemia
- phenochromocytoma
pregnant: Only with clear indication. Asthma control is more important than theoretical risks.
Style time: Probably safe for inhalative use.
Possible Side Effects
Acid (1-10%):
- Tremor
- Heart palpitations, tachycardia
- Headaches
- Muscle cramps
Occasionally:
- Unrest, nervousness
- Sleep disorders
- Mouth dryness
- nausea
Selten:
- Heart rhythm disorders
- Hypokaliemia (at high doses)
- Paradox bronchospasmus
- Allergic reactions
** Side effects are significantly lower in inhalative use than in oral administration.
Interactions
Reactions:
- Betablockers – can eliminate formoterol action
- Avoid non-selective beta blockers at Asthma!
- QT-extending medication – increased arrhythmia risk
- MAO inhibitors, tricyclic antidepressants – enhanced heart action
- Corticoids, Diuretics – Hypokaliemia Risk
Ingenious combination:
- Inhalative corticoids (forcing with asthma)
- Anticholinergics (e.g. tiotropium)
