What is Valaciclovir?
Valaciclovir is a prodrug that is converted into aciclovir in the body. It offers the same antiviral action against herpesviruses, but is better absorbed by the body. As a result, higher levels of action can be achieved, and taking is simpler (less tablets per day).
The drug is used for the treatment and prophylaxis of Herpes genital, lips and belt rose. In genital herpes, Valaciclovir is often the preferred choice because of the more convenient dosage scheme.
Valaciclovir is known as Valtrex®.
Active Ingredients & Mechanism of Action
Active ingredient: Valaciclovir
Valaciclovir is quickly transformed into Aciclovir in the body:
Pharmacokinetics:
- After oral administration, Valaciclovir is converted to 54% in Aciclovir (biover availability)
- Compared: Aciclovir tablets have only 15-30% bioavailability
- Higher Aciclovir levels possible in blood
Active mechanism (identical with Aciclovir): 1. Activation by viral thymidine kinase Two. Inhibition of viral DNA polymerase 3. DNA chain termination 4. Stop virus reproduction
Advantages over Aciclovir:
- Better absorption from the intestine
- Higher active substance levels
- Less tablets per day
- Better compliance
Who is it suitable for?
Valaciclovir is suitable for:
- Genital herpes (first infection and recurrence)
- Suppression therapy for frequent genital herpes
- Lips
- Belt rose
- Reducing transmission risk to partners
Advantages:
- Simpler use than Aciclovir
- Good for long-term prophylaxis
- Proven reduction of partner transmission
Restrictions such as Aciclovir:
- Don't heal herpes
- Should be started early
- dose adjustment for kidney failure
Available Dosages
Genital herpes – First infection:
- 500 mg 2x daily for 10 days
Genital herpes – Rezidiv:
- 500 mg 2x daily for 3-5 days OR
- 1000 mg 1x daily for 3-5 days
Suppression therapy:
- 500 mg 1x per day (at <10 rezidives/year)
- 1000 mg 1x daily (at ≥10 recurrences/year)
- 500 mg 1x per day for reduction of transmission
Lippenherpes:
- 2000 mg 2x daily for 1 day (12 hours distance)
Gurtelrose:
- 1000 mg 3x daily for 7 days
Available starches:
- 500 mg tablets
- 1000 mg tablets
How to Take
Intake:
- With plenty of water (at least 200 ml)
- Independent meals
- For multiple doses: uniform intervals
In case of acute outbreak:
- Start as early as possible
- Ideally at first creeping
- The sooner, the better the effect
Suppression therapy:
- Every day at about the same time
- Do not forget – regular intake important
- After 6-12 months of exhaust test possible
Important: Sufficient drink during treatment to protect the kidneys.
Contraindications
Do not apply at:
- hypersensitivity to Valaciclovir or Aciclovir
Dosis adjustment at:
- Kidney insufficiency (important!)
- Older patients
- Dehydration
pregnant and breastfeeding: Can be used with appropriate indication. Long-standing experience, no known risks for the child.
Possible Side Effects
Acid (1-10%):
- Headaches
- nausea

- photos sensitivity
Selten (especially at high doses/renal failure):
- Neurological symptoms (confusion, hallucinations)
- Kidney problems
- Thrombotic microangiopathy (in immunosuppressed patients)
* *Compared to Aciclovir:** Similar side effect profile, since Valaciclovir is converted into Aciclovir.
Interactions
Reactions:
- Nephrotoxic medicines – additive kidney stress
- Mycophenolate - raised mirror
- Cimetidine, sample ecide – delayed excretion
For renal insufficiency: Dose reduction is required because Aciclovir is excreted over the kidneys.
No relevant influence:
- Hormonal contraception
- Custom everyday media
